You may email me at klhodges at ou.edu to set up an appointment or for other business; I will usually answer promptly if it is urgent.
If you want to drop off a form or a late paper for me, it is best to put it in my mail box in the main English office (on the first floor of Gittinger). Sliding it under my door is not such a good idea. Remember, if you're tempted to turn in a paper this way and not in class, what determines how late your paper will be marked is when I receive your paper, not when you drop it off.
