INFORMED CONSENT FORM FOR RESEARCH BEING CONDUCTED UNDER THE AUSPICES OF THE UNIVERSITY OF OKLAHOMA-NORMAN CAMPUS
This study, entitled: "Social Networks and Social Support " is concerned with why people choose certain types of individuals to discuss their problems and/or seek various types of social support. This research is being directed by Dr. Kevin Wright. If you participate, you will be asked to complete a survey questionnaire asking about the types of people you communicate with when you are dealing with problems and types of support you receive from these individuals. Your participation should take between 10 - 15 minutes.
RISKS AND BENEFITS: There are no risks associated with this study beyond what would be expected by chance. In addition, the study will provide new information about social support networks. The researchers plan to construct a website that details the findings of the study (using anonymous and aggregate data) for participants who may be interested in the results.
CONDITIONS OF PARTICIPATION: Participation is voluntary. Refusal to participate will involve no penalty or loss of benefits to which the subject is otherwise entitled. Furthermore, the participant may discontinue participation at any time without penalty or loss of benefits to which the participant is otherwise entitled.
CONFIDENTIALITY: Findings will be presented in aggregate form with no identifying information to ensure confidentiality. Participants' names will be linked with their responses unless the participant specifically wishes to remain anonymous.
